What are my payment options?

We accept cash, credit/debit including HSA or FSA, Venmo, Zelle or Google Pay; weekly payments can be arranged for private pay packages.

Payment for services is due upon booking or upon arrival for your apointment. When booking an appointment you will be required to create an account with us. There you are able to store a credit card securely in your profile to use at your convenience. You do not need to use that card to pay for services as we do accept other forms of payment as noted above.

 

Are you in network with insurance providers?

B Well Integrated Physical Therapy now accepts Blue Cross & Blue Shield of Michigan and Medicare as well as Advantage & Supplemental Plans. Please inquire about other insurances with out-of-network benefits, auto claims & worker's compensation claims. 

 

What if I don't have insurance coverage?

Private pay is always accepted and those prices are listed on the booking page. Don't let lack of health insurance coverage stand in the way of having the kind of treatment you deserve! We also consider financial hardship discounts and payment programs, so please reach out using the "Contact Us" form on the home page so we can discuss an option that works best for you. It is our mission to provide the very best physical therapy for all.

 

Can I submit this cost to my insurance provider for reimbursement?

For those with insurance coverage that we are out-of-network with (please contact us to confirm):

If your injury and/or treatment meet insurance provider criteria for coverage (medical necessity), and your policy has out of network (OON) benefits, a detailed invoice (Superbill) will be created for you to send to your provider for reimbursement directly to you. However, payment for services is required at the time of your visit.

 

Do I need a prescription from my physician?

If you are using your insurance to cover the cost of your therapy, we can evaluate your injury and develop a plan of care to help you recover without a prescription. All insurance companies require a physician signature on that plan of care to cover our services and we will discuss with you the best route to accomplish that.

If you are not using insurance to cover the cost of your therapy, Michigan law allows us to evaluate and treat self-referring patients without a prescription from a physician under the following circumstances: (1) for up to 21 days or 10 treatments, whichever comes first; or (2) the patient is seeking physical therapy services to prevent injury or promote fitness.
